Step 2 CK
Step 2 Clinical Knowledge is the scored exam residency programs weigh most heavily — the one three-digit number that filters interview invitations. Since Step 1 went pass/fail in January 2022, Step 2 CK has absorbed the stratification role the old scored Step 1 once played.
Step 2 CK assesses clinical knowledge — the ability to apply medical science to patient care: diagnosis, management, and the principles of clinical reasoning across disciplines. Where Step 1 asks whether you understand the foundations, Step 2 CK asks whether you can use them at the bedside. It is taken during the clinical years, typically after a meaningful block of core clerkships so that the vignette-style questions land on experience, not just reading.
When Step 1 reported a score, many programs used it as the first screen for interview invitations. With that signal gone, Step 2 CK became the primary quantitative comparison between applicants — the only scored USMLE result available before residency selection for most US students. Students now face a different timing calculus: a strong Step 2 CK score needs to be in the application file when programs make interview decisions, which has shifted when many students choose to take it.
The national mean score was 249 across 2023–24, and the passing standard was raised to 218 in July 2025. First-time pass rates are high and stable: 98% for US MD takers and 96% for US DO takers across the 2022–2024 cycles — but the pass rate tells only part of the story. Because programs use the score to compare candidates, the distribution matters more than the pass line: small score differences can move an applicant across a program's screening threshold.
Residency stakes: In the 2024 NRMP program-director survey, 77% of respondents said they consider failed Step attempts when deciding whom to interview. A first-time pass on Step 2 CK — with a competitive score — is one of the highest-leverage items in an application file.
